Acknowledgement of Policies, Payment, & Liability Waiver

General Information:

Physician's clearance is required prior to testing or exercise if you are 45 or older and in poor physical health.

The Coach cannot diagnose or treat medical injuries or illness or prescribe medication.


Common food facts may be discussed. 

If possible a 24 hours’ notice is appreciated if you plan to cancel a session. The Coach understands this is not always possible and is understanding of special circumstances. Clients who do not cancel within a 24 hour
 notice will forfeit their session and be subject to a cancellation fee.

All sessions are private, one-on-one training.

Duration of sessions are 30 or 60 minutes. 

If you are late for a session, you may receive only the time remaining. 

 

We accept Paypal, major credit cards, check and cash. 

 

PLEASE NOTE: Checks returned for insufficient funds will be subject to a $25.00 fee.

Recommendation:


My recommendations are at least 2 days a week and these sessions should be used for meal planning advice, accountability, exercising assistance, and questions regarding your workout and wellness program.

Remember, the coach is here for you to guide you on the path to better health!

You should be aware and acknowledge that results are not guaranteed and will vary; your diet, discipline, effort and other factors outside the trainer's control will influence your success. If you forfeit your sessions, there will be NO refund for the services the coach was willing to provide.

In order to assess your general health risk and tailor the training process to your individual needs, you will be asked questions about you, and your family's, health history. Your privacy is respected. Your information will never be shared without your advanced written consent, and will not be used for any purpose other than adjusting your fitness training program and assessing your health risk as it pertains to your training. An educated trainer is better able to help you safely meet your goals. 

In addition, as part of the assessment process, you will be asked to perform one or more tests to measure your level of fitness in several categories (i.e., cardiopulmonary, flexibility, strength, endurance and power). This testing can be as strenuous as exercise and will recur as needed to gauge your progress. Results are compared to norms and your prior results to measure success and keep you motivated.

No activity between Laurie Gouley and Client creates a doctor-patient relationship. No activity between Laurie Gouley and Client should replace or be deemed to replace the formal advice or services of a licensed healthcare professional in your state. Accordingly, you should not rely on any material Laurie Gouley teaches for specific matters relating to your individual health care needs. 

All discussion between client and coach are confidential.

You should consult your physician or other health care professional before starting this or any other fitness program to determine if it is right for your needs. This is particularly true if you (or your family) have a history of high blood pressure or heart disease, or if you have ever experienced chest pain when exercising or have experienced chest pain in the past month when not engaged in physical activity, smoke, have high cholesterol, are obese, or have a bone or joint problem that could be made worse by a change in physical activity. Do not start this fitness program if your physician or health care provider advises against it. If you experience faintness, dizziness, pain or shortness of breath at any time while exercising you should stop immediately. 

It is recommended that you eat a balanced, light meal or energizing snack 1-2 hours prior to any workout and that you drink plenty of water all day to stay hydrated. When exercising, wear sneakers and loose-fitting and/or supportive clothing (as needed) in which you will be comfortable during the motions and positioning that are part of the training sessions.

In consideration of my participation in this program, I, the undersigned, hereby waive and release Laurie Gouley, her successors and assigns, from any and all claims, costs, liability and expense for any injury, loss or damage whether known, anticipated or unanticipated arising from my voluntary participation and enrollment in these sessions.
